HERE IS A LIST OF DOCUMENTS WE ASK YOU TO PREPARE FOR OUR FIRST MEETING

  1. A monthly budget of your expenses, Click here to download a copy of the form.
  2. Mutual Funds/Stock/Bond etc. statements (Preferably the recent year end)
  3. Latest bank statement-listing accounts
  4. Any pension or 401(k) / 403b/457K plan statements
  5. Recent IRA statements
    (Traditional and/or Roth)
  6. Recent Social Security Statements
  7. Copy of recent Mortgage Statement
  8. Tax returns for the last 2 years
  9. Wills and trust documents
  10. All insurance policies, including life,
    disability, long-term care
  11. Company provided group benefits for you and your spouse.
    (If a print-out of specific coverages and benefits are available, please include those)
